Incident investigations

Rio Tinto Coal Australia is committed to creating an injury and illness free workplace where everyone goes home safe and healthy every day.  Thoroughly investigating safety incidents and determining their root causes is key to improving our safety performance.

Currently, root causes from safety incidents ranked 'high' are communicated across the Rio Tinto Coal Australia business, with the primary objective being to share key learnings across the business.

In 2007 a strategy was developed to extend this approach to include reporting on incidents ranked 'medium' and 'low'.  This will be made possible by the launch of a new business solution which will provide a common system within Rio Tinto Coal Australia for the management of health and safety data. 

Once this system is in place, detailed monitoring of all safety incidents will take place across Rio Tinto Coal Australia, with the ultimate objectives being to improve incidents investigations across Rio Tinto Coal Australia and to identify, learn and share lessons from root causes.
